Every working day in Mumbai (which we used to mispronounce Bombay), India, 5000 illiterate / semiliterate couriers deliver 175,000 box lunches to workers, carrying them on bikes, shoulder trays, etc. They have a total error rate of 1 mistake (late or wrong lunch) every two months to their subscribers, achieving 1 error per six million transactions (i.e. far better than "Six Sigma method" performance):
